The Leaving Life Questionnaire is about death and dying . Some of the topics covered are: what you learned about death and dying as you were growing up, your ability to adapt to very old age,  and your thoughts about the social consequences of making leaving life (or voluntary euthanasia) more accessible.    You are asked about factors that might affect a personal decision, such as quality of life or level of suffering, the reactions of others to your dying, your value to society, and other factors.
